Giveaway Terms and Conditions

By participating in the Promotion, entrants agree to these Terms & Conditions in their entirety.

1. Eligibility: Participation in Ventra Health’s giveaway promotion (the “Promotion”) is open to legal residents of the United States who are 18 years of age or older at the time of entry. Proof of age and identity may be required. Participants must meet all entry requirements outlined in these Terms & Conditions to be eligible. Employees of Ventra Health, their immediate family members (spouse, partner, parent, child, sibling), and anyone associated with Ventra Health are not eligible to enter. The Promotion is void where prohibited by law.
2. Promotion Overview: This Promotion is a contest where participants submit entries for potential selection by Ventra Health’s designated representatives. Winner selection is discretionary and based on subjective criteria. Participation is entirely voluntary, and no purchase or payment is necessary to enter or win.
3. Sponsor: The Promotion is sponsored by Ventra Health (the “Sponsor”).
4. Entry Method: Participants may enter by completing the official entry form at Ventra Health trade show booths or by submitting the online entry form at the designated website or URL. Each person is limited to one entry per email address during the Promotional Period. Duplicate entries, automated entries, or entries submitted through unauthorized methods will be void.
5. Entry Requirements: Valid entries must include complete, accurate, and verifiable information, including practice name, first name, last name, title, email address, and phone number. Incomplete or inaccurate submissions may be disqualified at Ventra Health’s sole discretion.
6. Selection of Winner: Winner selection will be conducted by Ventra Health's representatives through a comprehensive review process. Entries will be evaluated based on various factors, which may include the completeness of information, potential business alignment, and overall quality of submission. Ventra Health reserves the right to select winners using a method that best serves the objectives of the Promotion.
7. Prizes: Prize details, including type and value, will be specified for each individual Promotion. All prizes are non-transferable, not exchangeable for cash, and subject to manufacturer warranties and any applicable restrictions. Ventra Health reserves the right to substitute a prize of equal or greater value if the original prize becomes unavailable for any reason.
8. Winner Notification: Potential winners will be notified via email or phone within ten (10) business days of winner selection. Winners may be required to complete and return an affidavit of eligibility and a liability/publicity release. Failure to respond within ten (10) business days may result in forfeiture of the prize, and an alternate winner may be selected.
9. Tax Responsibilities: Winners are responsible for all taxes associated with prize receipt. Prizes valued over $600 will require a 1099 tax form.
10. Publicity Release: By participating, entrants grant Ventra Health, where permitted by law, the irrevocable right to use their name, likeness, and submitted information for promotional purposes in any media without additional compensation.
11. Limitation of Liability: Participants release and hold harmless Ventra Health, its affiliates, subsidiaries, and agents from any liability for damages, losses, or claims arising from participation in the Promotion or the receipt, use, or misuse of the prize. Ventra Health is not responsible for lost, late, incomplete, or misdirected entries, technical failures, or any disruptions beyond its control.
12. Conduct and Disqualification: Ventra Health reserves the right to disqualify any participant who tampers with the entry process, submits false information, violates these Terms & Conditions, or engages in any conduct deemed inappropriate, unsportsmanlike, or disruptive. All decisions by Ventra Health are final and binding.
13. Intellectual Property and Confidentiality: Submitted information may be used by Ventra Health for internal business purposes, including marketing and promotional outreach. Participants retain ownership of any original content submitted but grant Ventra Health a non-exclusive license to use it for promotional purposes.
14. Modification and Cancellation: Ventra Health reserves the right to modify, suspend, or cancel the Promotion at any time for any reason, including but not limited to fraud, technical issues, or any other factor that compromises the integrity of the Promotion.
15. Governing Law: This Promotion is subject to all applicable federal, state, and local laws and regulations. These Terms & Conditions are governed by the laws of the State of Texas. Any disputes will be resolved exclusively in the state or federal courts located in Dallas County, Texas.
